Pay Rate: $16.75
Summary Of Duties
Responsible for supporting counseling staff in the Certified Community Behavioral Health Clinic. Will provide supporting duties including requesting and organizing clinical/medical records, completing supportive documents for counseling staff, providing support to counselors as needed.
Responsibilities
Assists in coordinating communication regarding care between clinical team, prescribers, and nursing staff
Monitoring and contacting clients, to include those identified as high risk, with no recent contact or no next appointment scheduled
Reaching out to collaterals and outside providers (PCP, insurance companies, inpatient, and detox facilities) involved in client care to request information
Assists counselors in obtaining resources needed for group and individual sessions
Obtains toxicology and lab reports for counselors
Helps to complete court reports, completion of paperwork and other clinical data needed
Coordinates and schedules necessary appointments
Prepare/Stage documentation such as discharges, for counselors to complete
Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.
Performs all other duties as assigned.
Qualifications
High School Diploma or GED
Some knowledge/experience with coordinating and collaborating between multiple person(s) or entities
Ability and flexibility to learn and execute new tasks and processes
Ability to ask for support and/or clarification when needed
Great organization, problem-solving and time management skills
Able to thrive in fast-paced environments
BIPOC and Bilingual English/Spanish, Arabic, or other language strongly encouraged to apply.
